Responsibilities

• Provide telephone-based assessment, triage, counseling in a 24/7 call center.
• Deliver risk, substance abuse and mental health assessments with safety plans.
• Collaborate with care team to ensure continuity of care.
• Maintain confidential documentation meeting risk management standards.
