The role supports continuity of care across our CenterWell Home Health footprint and requires precise data handling and timely communication. The position is full-time, primarily remote with occasional travel to Humana offices for training or meetings.

Qualifications

• High School Diploma or its equivalent is required.
• Typing speed of 40 WPM or more.
• Ability to comprehend and apply basic math for data analysis and reporting.
• Proficiency with Microsoft Office applications (Word, Excel, Outlook, Teams).
• Good logical thinking and decision-making skills.
• Ability to follow strict timelines.
• Effective communication with all levels of management.
• Ability to work with multiple screens/systems with high attention to detail.
• Strong time-management skills in a fast-paced environment.
• Must read, write and speak fluent English.
• Regular attendance is required.

Responsibilities

• Monitor multiple e-referral systems to identify new referrals and manage alerts.
• Monitor email for requests to pull referrals and perform follow-up communications.
• Scan documentation to determine patient eligibility for services.
• Track referral status and provide weekly status reports.
• Communicate with branch, sales, and leadership for status updates and decisions.
• Daily management of the ERST log with referrals and ROC decisions.
• Maintain acceptable productivity according to assignments and standards.
• Adhere to regulatory compliance policies and procedures.
