Job summary

Millennium Physician Group is seeking a Discharge Planner I for its Hospitalists program. This fully remote, full-time role operates on Monday–Friday EST hours and focuses on coordinating safe, timely hospital discharges and post-acute services.

You will assess patient needs, develop individualized discharge plans, collaborate with physicians, nurses and case managers, educate patients and caregivers, and ensure documentation complies with regulatory standards and organizational policies.

Qualifications

• Assess discharge planning needs through review of records and interviews.
• Develop and coordinate individualized discharge plans.
• Collaborate with hospitalists, nurses, case managers, therapists, pharmacists and social workers.
• Arrange post-acute services such as home health and SNFs.
• Educate patients and caregivers on discharge instructions and follow-up.

Responsibilities

• Assess patients' discharge planning needs through review of medical records, interdisciplinary collaboration, patient interviews, and evaluation of factors affecting post-discharge care.
• Develop and coordinate individualized discharge plans that align with physician orders and payer requirements.
• Collaborate with hospitalists, nursing staff, case managers, and therapists to coordinate timely, safe discharges.
• Arrange post-acute services including home health, SNF, rehab, equipment, hospice and transportation as appropriate.
• Educate patients and caregivers regarding discharge instructions, medications and follow-up care.
