Responsibilities

• Assess discharge planning needs through medical record review and patient interviews.
• Develop individualized discharge plans aligned with physician orders and payer requirements.
• Coordinate with hospital teams to ensure timely, safe discharges and post-acute services.
• Educate patients and caregivers on discharge instructions and follow-up plans.
• Identify barriers to discharge and coordinate interventions to overcome them.
