Job summary

Millennium Physician Group seeks a Discharge Planner I for Hospitalists to coordinate discharge planning for hospitalized patients. This is a full-time, fully remote role based on EST hours, Monday through Friday.

You will assess needs, coordinate post-acute services, educate patients and families, and collaborate with physicians, nurses, case managers, therapists, social workers and community providers to promote safe, timely transitions and reduce readmissions.

Qualifications

• Discharge planning experience preferred in hospital setting.
• Ability to coordinate with physicians, nurses, case managers, patients and families.

Responsibilities

• Assess discharge planning needs through medical records, interviews and environment.
• Develop and coordinate individualized discharge plans aligned with physician orders and payer requirements.
• Collaborate with hospital staff to coordinate timely, safe discharges.
• Arrange post-acute services including home health, SNF, rehab and equipment as appropriate.
• Educate patients and caregivers on discharge instructions, medications and follow-up.
