Job summary

UnitedHealth Group is seeking a Hospital Safe Discharge Case Manager to proactively coordinate discharge planning across acute care settings. You will work with facility CM/UM staff, medical directors, and care teams to resolve barriers and ensure timely transitions of care.

The role supports remote work from anywhere in the U.S. and requires 5+ years of nursing with discharge planning experience, strong communication, and proficiency with EMR systems and care management tools.

Qualifications

• RN licensure and a BSN degree required.
• 5+ years of nursing experience in an acute care setting.
• 2+ years of discharge planning experience.
• Knowledge of utilization management and quality improvement.
• Proficiency with Microsoft Office and EMR systems.

Responsibilities

• Assess barriers to safe, efficient discharge and coordinate resources.
• Demonstrate knowledge of case management and care coordination for transition planning.
• Manage caseload efficiently with time management.
• Lead rounds to discuss needs and prioritize discharges.
• Document clinical information accurately in care management systems.
