Job summary

Medrina is hiring a Director of Clinical Care Management to lead CCM and PCM programs across markets from a remote home office in the central or eastern United States, with occasional travel. The role ensures standardized workflows, regulatory compliance, enrollment processes, and value-based outcomes.

The candidate will oversee nursing and care-management staff and coordinate with physicians, facilities, and revenue teams.

Qualifications

• Registered Nurse with active license and leadership experience.
• Experience managing chronic/complex patient populations.
• Experience leading clinical teams and workflows.

Responsibilities

• Provide clinical operations leadership for CCM and PCM services.
• Develop standardized care-management workflows across markets/facilities.
• Establish clinical standards and escalation protocols with physicians.
• Ensure regulatory documentation and compliant workflows.
• Oversee enrollment, education, and communication with facilities and providers.
• Monitor KPIs and drive program performance and value.
