Job summary

Medrina is seeking a Director of Care Transitions and Strategic Partnerships to oversee the FastPAC and FastPAC Plus programs. The role emphasizes strategic growth, operational performance, analytics, and executive-level reporting across hospitals and post-acute providers.

You will identify opportunities to expand programs, partner with clinical and operational leaders, and translate data into actionable insights for senior stakeholders.

Qualifications

• Bachelor's degree required in a healthcare or related field.
• Strong analytical skills and ability to interpret data.
• Excellent written, verbal and executive communication skills.
• Proven track record in developing dashboards, reports, and presentations.
• Strong project-management and program-development capabilities.

Responsibilities

• Provide overall leadership for FastPAC and FastPAC Plus programs, ensuring consistent execution.
• Develop strategies to expand FastPAC and FastPAC Plus across hospitals, SNFs, markets and payer relationships.
• Coordinate with hospital teams, Medrina providers, care managers and partners to improve transitions of care.
• Develop executive dashboards, reports and summaries to communicate performance to senior leadership.
