Job summary

Qualifications

• Minimum 1 year in remote patient monitoring or chronic care management.
• Understanding CPAP therapy and common issues like leaks.
• Strong documentation and data-driven communication.

Responsibilities

• Review CPAP data daily and flag patients needing outreach.
• Conduct outbound calls with scripted guidance and empathy.
• Document calls in Sanusom and Athena for billing accuracy.
• Coordinate with DME and CPAP supply teams.
• Escalate clinical concerns per protocol.
