Job summary

TCH Medical Center is seeking a Social Worker to assess, plan, implement, monitor and evaluate care to meet health needs of Health Plan members. You will coordinate and manage care for high-risk members across the continuum, ensuring quality, cost effectiveness and patient satisfaction.

Qualifications

• Master's Degree in Social Work is required.
• LMSW, LCSW, or LMSW-AP licensure by TBHEC is required.

Responsibilities

• Assesses, develops, implements and monitors a comprehensive plan of care through an interdisciplinary team process with the member/family.
• Identifies members in need of case management through rounds, referrals, and coordination with staff and PCPs.
• Assesses biophysical, psychosocial, environmental, discharge planning needs and financial status.
• Plans and coordinates services across the continuum of care and documents it in the system.
• Advocates for members and families across the continuum of care.
