Job summary

Medica is seeking an RN Case Manager to deliver patient-centered care across multiple products. You will collaborate with members, families, caregivers, providers and interdisciplinary teams to assess needs, develop, implement and monitor individualized care plans, and coordinate services across the healthcare continuum.

The role requires an RN license and an Associate's degree; CCM preferred or obtainable within two years.

Qualifications

• Associate's degree in Nursing (or equivalent) is required.
• Current, unrestricted RN license in the state of residence.
• CCM certification preferred or ability to obtain within two years.

Responsibilities

• Conduct comprehensive assessments and develop individualized care plans.
• Coordinate services across the healthcare continuum.
• Monitor and adjust care plans to reduce illness burden and costs.
