Job summary

Lyra Health is seeking a Clinical Specialist for its Complex Care program to assess client needs for higher levels of care, provide in-the-moment phone support, ensure safety, and guide clients to high-quality, evidence-based facilities and programs. This remote role focuses on connecting clients to care via phone and email with longitudinal case management.

Qualifications

• Master's degree in clinical social work or equivalent required.
• Independent licensure (LCSW/LMFT/LPCC/LPC) required; supervised hours not provided.

Responsibilities

• Independently manage a caseload of complex, multi-need clients with long-term case management.
• Support clients through various levels of care using Motivational Interviewing and other strategies.
• Respond to crises with risk and needs assessments, safety planning, and de-escalation.
• Communicate with clients and providers via email, phone, and text as needed.
• Coordinate with facilities, health plans, and navigators to ensure access to treatment.
