Job summary

Lyra Health, Inc. is seeking a Clinical Care Navigator (LCSW) for a remote, high-acuity clinical role within its fast-paced platform. You will assess risk, provide brief interventions, crisis support, and connect clients with appropriate care through Lyra's tech-enabled services.

The role requires licensure (LCSW or equivalent or RN with behavioral health experience), full-time US residency, and offers a base rate of $38.65/hour plus a 10% weekend differential, with comprehensive benefits.

Qualifications

• Master's degree in clinical social work or equivalent; independent licensure required (e.g., LCSW, LMFT, LPC, LPCC) or RN.
• Two years of behavioral health experience for RNs; post-license experience required for other licenses.
• Significant experience managing high-acuity clinical situations (e.g., suicidality/homicidality).
• Bilingual Spanish preferred but not required; candidate must be a full-time US resident.

Responsibilities

• Provide compassionate, timely clinical support including crisis and elevated-risk situations.
• Conduct risk assessments and safety planning; de-escalation and evidence-based coping interventions.
• Provide brief support across mental health, substance use, relationships, work stress, and life challenges.
• Assist clients in connecting to Lyra resources and care options.
• Utilize Lyra platforms and provider network to facilitate access to services.
• Document interactions clearly and timely; follow internal protocols.
• Collaborate with Lyra teams to ensure safe, effective client care.
