Lyra Health, Inc. is seeking a Licensed Clinical Care Navigator to provide high-quality, remote clinical support for mental health clients in the United States.

The role blends clinical practice with digital health tools and requires a full-time US resident with independent licensure or RN with behavioral health experience. You will conduct risk assessments, provide brief interventions, and connect clients to Lyra resources and providers.

Formación

• Master's degree in clinical social work or equivalent; independent licensure required (LCSW/LMFT/LPC/LPCC) or RN with behavioral health experience.
• Two years of behavioral health experience; post-license experience required for non-NP licensures.
• Significant experience managing high-acuity clinical situations including crisis risk assessment.

Responsabilidades

• Provide compassionate, timely clinical support to clients with emotional, behavioral, and situational concerns.
• Conduct risk assessments and safety planning; de-escalation and coping interventions.
• Connect clients with Lyra resources and care options aligned to clinical needs.
• Utilize Lyra platforms and provider networks to access services efficiently.
• Document interactions and follow-up; maintain compliance with internal protocols.
• Collaborate with Lyra teams to ensure safe and seamless client care.
