Job summary

Upperline Health is seeking a remote Care Navigator to support clinicians in delivering complex care plans for Upperline Plus patients. You will coordinate telephonic care, schedule visits, and manage referrals while working with a multidisciplinary team to improve patient outcomes.

Ideal candidates have healthcare experience with chronically ill patients, strong communication, and organizational skills. Remote role, Monday–Friday, CST hours.

Qualifications

• Associates degree or higher education required
• BSW, CNA, LPN, or MA certification highly required
• Experience with chronically ill patients preferred

Responsibilities

• Conduct telephonic care coordination with patients and families.
• Develop and manage patient care plans with monthly reviews.
• Serve as liaison between patients, families and providers.
• Provide education and resources to patients to navigate care.
• Coordinate scheduling for in-home, clinic, and telehealth visits.
• Participate in weekly interdisciplinary team meetings.
• Perform related clinical and administrative duties as needed.
