The Coordinating Center in Maryland is seeking a full-time Coordinator of Community Services to deliver targeted case management for people with intellectual/developmental disabilities. You will connect clients with medical and community supports and provide person- and family-centered coordination under the CCS contract.

Responsibilities include maintaining a 30–40 client caseload, advocating for client goals, developing PCPs, and ensuring timely access to services and documentation.

Qualifications

• Bachelor’s degree or Associate with 2 years of human service experience or 7 years of relevant work experience.
• Experience coordinating services for Medicaid/waiver or IDD supports preferred.
• Competency in negotiation, advocacy, relationship-building, and person-centered care.

Responsabilités

• Maintain a caseload of 30–40 clients with IDD.
• Advocate for clients and support self-advocacy.
• Develop and implement Person-Centered Plans per COMAR and policies.
• Access and coordinate services to meet client needs in integrated settings.
• Document services and ensure billing requirements are met.
• Visit clients at least four times per plan year or as needed.
• Coordinate PCPs, quarterly visits, and interdisciplinary meetings.
• Conduct assessments per DDA regulations and program requirements.
• Ensure clients know provider options and rights to choose services.
• Respond to incident reports to safeguard health and safety.
