• Willing to travel up to 25% within assigned regions to visit Medicaid members.
• Must reside in Nebraska with a valid Nebraska driver’s license and current automobile insurance.

Responsibilities

• Assess, plan, and implement individualized care strategies for members.
• Create a positive relationship with members and coordinate with care teams.
• Identify and initiate referrals for social service programs and resources.
• Document all member assessments, care plans and referrals.
• Communicate required health information to stakeholders for coordinated care.
