Job summary

Qualifications

• Requires High School diploma or GED.
• 1–2 years of related experience.
• Knowledge of community resources and health education.

Responsibilities

• Supports community connection activities and connects members to known community resources to support their care management journey.
• Conducts non-clinical general health assessments to refer members to appropriate care/services.
• Conducts telephonic and in-person outreach to locate individuals and families in the community.
• Documents and maintains all community resources to ensure compliance with health plan policies.
• Assists the clinical team of nurses and social workers.
