Imagine Pediatrics is actively seeking a Community Health Worker, Patient Engagement to re‑engage patients with our pediatric care services. The role operates remotely from Knoxville, TN with frequent travel across the area, partnering with an interdisciplinary team to support onboarding, care access, and community connections.

You will educate families about services and local resources while coordinating with behavioral health teams and care providers.

Qualifikationen

• 3+ years of experience in a healthcare setting (patient-facing hospital or facility role).
• Community Health Worker (CHW) certification preferred.
• Valid driver’s license and vehicle; ability to travel frequently.
• Medical Interpreter Certification preferred.

Aufgaben

• Serve as a community-based, patient-facing resource, building relationships with patients and caregivers through direct, in-person re-engagement in assigned geographic areas.
• Provide hands-on support to patients and caregivers, including assisting with program re-enrollment, onboarding, and mobile app setup and utilization.
• Perform telephonic outreach to caregivers of attributed patients to re-engage children in care and support onboarding.
• Educate patients and caregivers on Imagine Pediatrics services and local resources for access to care and support.
• Maintain up-to-date knowledge of local community resources (SDOH, social services) and connect families to appropriate services.
• Conduct community events and facility rounding to engage eligible patients and support ongoing participation.
