Job summary

Imagine Pediatrics is seeking a remote-based Patient Engagement role in Chattanooga, TN, with frequent travel within the area. The job focuses on re-engaging patients, onboarding, and connecting families to community resources through collaboration with an interdisciplinary team.

You will support program enrollment, provide in-person and telephonic outreach, and educate families about services while navigating social determinants of health.

Qualifications

• 3+ years of experience in a healthcare setting (patient-facing).
• Community Health Worker (CHW) certification required.
• A valid driver’s license and vehicle.
• A bachelor’s degree.
• Ability to travel frequently.
• Bilingual Spanish preferred.
• Medical Interpreter Certification preferred.
• Ability to navigate ambiguity, think critically, take initiative, and stay organized.
• Ability to manage multiple tech platforms.
• Experience in a patient-facing hospital or facility role.

Responsibilities

• Build relationships with patients and caregivers through direct, in-person re-engagement.
• Provide hands-on support for program re-enrollment, onboarding, and mobile app setup.
• Perform telephonic outreach to re-engage patients and onboarding.
• Educate patients and caregivers on Imagine Pediatrics services and local resources.
• Maintain knowledge of local community resources and connect families to services.
• Conduct community events and facility rounding to engage patients.
• Partner with Behavioral Health teams for discharge planning and connections to resources.
• Collaborate with caregivers, families, and facility staff to address barriers to care.
• Coordinate with Outreach Specialists and Registered Nurses on re-engagement goals.
• Support non-clinical care coordination in the community (primary care, pharmacy access, discharge follow-up, SDOH).
