You will use peer coaching strategies, engage in training, and collaborate with staff. This position offers work-from-home flexibility with local events as needed.

Qualifications

• Must be located in Santa Clara County.
• Certificate pathway or 2,000 hours CHW experience required.
• Reliable transportation and flexible schedule for outreach.
• Excellent written and verbal communication; engages staff and community partners.
• Strong computer skills and ability to use state-required programs.

Responsibilities

• Carry out intake and assess health and social needs by phone.
• Follow processes and policies with accuracy and timeliness.
