Job summary

Qualifications

• Travel 60-75% with reliable transportation.
• 3–5 years of relevant work experience.
• 1–2 years of clinical support experience.
• 1 year of community outreach experience.

Responsibilities

• Conduct home visits to reinforce care plans and adapt tasks to culture and language.
• Educate providers on social determinants of health and care opportunities.
• Build relationships with members, providers, and staff from diverse backgrounds.
• Assist with care plan implementation and health management.
• Identify socio-economic issues affecting health and develop plans.
• Assist with linking to community services and scheduling appointments.
• Support medication adherence and disease self-management.
• Enter and maintain records in the EHR and complete documentation.
• Travel extensively to client homes and outreach destinations.
