Qualifications

• Must reside in LA and within the assigned region: Region 9 (St. Tammany, Livingston, Washington, Tangipahoa).
• If not CHW certified at time of hire, must complete CHW/Promotora certification within 12 months.
• Ability to travel 60-75% of the time; must have reliable transportation.
• 3-5 years' relevant work experience.
• 1-2 years' experience providing clinical support services.
• 1 year of community outreach experience and knowledge of LA resources.
• 1 year' experience working with clients with clinical needs (physical and/or behavioral health).
• Proficiency with MS Office Suite and EHR systems.
• Flexibility to work outside core hours; 2 days weekly until 9pm local time.
• Variable work schedule.

Responsibilities

• Conduct home visits with members to reinforce care plans and adapt tasks to culture and language.
• Educate healthcare professionals on social determinants of health impacting care.
• Build and maintain relationships with members, providers, case managers, and staff.
• Help reduce cultural and socio-economic barriers between clients and institutions.
• Assist with care plan implementation and education during in-home visits.
• Develop health and social management plans with members.
• Identify gaps to strengthen care systems and coordinate services.
• Assist with medication adherence and refills.
• Teach disease self-management including nutrition and symptom tracking.
• Enter and maintain member records in EHR; complete reports and documentation.
• Travel extensively to client homes and outreach destinations.
