Job summary

CVS Health Corporation is seeking a Community Health Worker to serve Louisiana communities in a full-time remote position. The role involves home visits, care plan support, and education across diverse populations, with 60-75% travel to member homes and providers.

While working under supervision, you will help identify social determinants of health, coordinate services, and document care activities in electronic health records. A Louisiana focus and regional travel define this essential role.

Qualifications

• Must reside in LA within Region 1 (Orleans, Jefferson, St. Bernard, Plaquemines).
• Travel 60-75% of the time with reliable transportation.
• 1-2 years of clinical support experience.
• 1 year of community outreach and knowledge of Louisiana resources.
• Experience working with clients with clinical needs (physical and/or behavioral health).
• Proficient with MS Office and electronic health record systems.
• Flexible hours outside core business times; some evenings/weekends may be required.

Responsibilities

• Conduct home visits to reinforce care plans and coordinate with member culture and needs.
• Educate providers on social determinants of health impacting care plans.
• Build relationships with members, providers, and office staff from diverse backgrounds.
• Assist with care plan implementation and health education during in-home visits.
• Identify socio-economic issues affecting health and develop management plans.
• Assist with scheduling and navigating social services and transportation resources.
• Support medication adherence and review current medications with members.
• Document interactions in electronic records and complete program reports.
• Travel extensively to client homes and outreach locations.
