• Experience with clients with clinical needs (physical and/or behavioral health).
• Proficiency with MS Office and basic computer skills.

Responsibilities

• Conduct home visits to reinforce care plans and address cultural needs.
• Educate providers on social determinants of health affecting care plans.
• Build relationships with members, providers, and staff across backgrounds.
• Assist with care plan implementation and health/social management planning.
• Document in electronic health record systems and prepare reports.
• Travel extensively to client homes and community locations.
