Job summary

Munson Healthcare is seeking a Community Health Worker to join our team serving northern Michigan. This remote position requires residency near Boyne, Charlevoix or Harbor Springs and offers a 40-hour work week, Monday–Friday, 8 a.m. to 5 p.m.

You will partner with patients, families, primary care teams, and community organizations to improve access to care and address social drivers of health. You will support patients through care coordination, resource navigation, outreach, and education,

Qualifications

• High School Diploma or GED required.
• Experience in health care, public health, community-based services, care coordination, behavioral health, or social services highly preferred.
• Knowledge of local community resources; Medicaid, rural populations, maternal-child health, or underserved communities a plus.
• Bilingual skills preferred where relevant.

Responsibilities

• The Community Health Worker serves as a trusted frontline public health worker partnering with patients, families, primary care teams, and community organizations to improve access to care and connect patients with resources.
• Assists with health system navigation, outreach, education, care plan reinforcement, and culturally responsive coaching. Does not provide clinical assessment, diagnosis, or treatment.
