Job summary

Munson Healthcare seeks a Community Health Worker to partner with patients, families, care teams, and community groups to improve access to care and connect patients with medical, behavioral health, and community resources.

The role supports health system navigation, outreach, education, and culturally responsive coaching, while not providing clinical assessments or services requiring licensure. Vaccination and regional residency requirements apply in Michigan.

Qualifications

• High School Diploma or GED is required.
• Experience in health care, public health, or related human services; knowledge of local community resources; Medicaid, rural populations, behavioral health, or underserved communities; bilingual skills preferred where relevant.
• Completion of, or willingness to complete, a Michigan-recognized CHW training program.
• MiCHWA CHW certification or eligibility is highly preferred.

Responsibilities

• Serve as a trusted frontline public health worker partnering with patients, families, primary care teams, and community organizations to improve access to care and connect patients with resources.
• Support patients through health system navigation, resource coordination, outreach, education, and culturally responsive coaching.
• Do not provide clinical assessments or services requiring licensure.
