Munson Healthcare seeks a Community Health Worker to collaborate with patients, families, primary care teams, and community organizations to improve access to care and link individuals to essential resources.

You will support care plan reinforcement, navigation within the health system, outreach, and education while coordinating community resources to address social determinants of health in northern Michigan.

Qualifications

• High School Diploma or GED is required.
• Experience in health care, public health, community-based services, human services, care coordination, behavioral health, or social services is preferred; bilingual skills may be a plus.
• Completion of, or willingness to complete, a Michigan-recognized CHW training program.
• Must live in the South Region of Northern Michigan.

Responsabilités

• The CHW serves as a trusted frontline public health worker who partners with patients, families, primary care teams, and community organizations to improve access to care, address social drivers of health, support chronic disease self-management, and connect patients with medical, behavioral health, and community-based resources.
• The CHW supports patients through health system navigation, resource coordination, outreach, education, care plan reinforcement, and culturally responsive coaching.
