Job summary

Munson Healthcare is seeking a Community Health Worker to collaborate with patients, families, and local care teams to improve access to care and connect people with community resources. The role emphasizes culturally responsive coaching and support across health services.

The CHW will assist with health system navigation, resource coordination, and education while avoiding clinical assessment or licensure tasks. Must live near Boyne, Charlevoix, or Harbor Springs in northern Michigan.

Qualifications

• Minimum high school education; experience in health care, public health, community-based services, human services, care coordination, behavioral health, or social services.
• Knowledge of local community resources; Medicaid/rural populations/underserved communities experience; bilingual skills preferred where relevant.
• Completion of, or willingness to complete, a Michigan-recognized CHW training program.
• MiCHWA CHW certification or eligibility is highly preferred.

Responsibilities

• Serve as a trusted frontline public health worker partnering with patients, families, primary care teams, and community organizations to improve access to care.
• Provide health system navigation, resource coordination, outreach, education, and care plan reinforcement with culturally responsive coaching.
