Job summary

Munson Healthcare is seeking a Community Health Worker to partner with patients, families, and primary care teams to improve access to care and address social determinants of health. The role focuses on resource coordination, outreach, education, and culturally responsive coaching, without providing clinical services.

You will support care plans, navigate health systems, and connect patients with medical, behavioral health, and community resources to enhance overall well-being.

Qualifications

• High School Diploma or GED required.
• Experience in health care, public health, community-based services, care coordination, behavioral health, or social services; knowledge of local resources; Medicaid, rural populations, behavioral health, chronic disease, maternal-child health, or underserved communities; bilingual skills preferred.

Responsibilities

• Serve as a trusted frontline public health worker, partnering with patients, families, primary care teams, and community organizations to improve access to care.
• Support health system navigation, resource coordination, outreach, education, and care plan reinforcement.
• Provide culturally responsive coaching; does not perform clinical assessment, diagnosis, or treatments.
