Qualifications

• Bachelor’s degree in social work, psychology, or a related field required.
• Minimum of 2–3 years of experience performing duties in DC, MD, or VA.
• Experience navigating social welfare systems.
• Proficient in virtual environment and comfortable engaging supervisory and team support.
• Proficient in Microsoft Office and experience with an electronic health record or similar technology.

Responsibilities

• Triage and assess needs stemming from economic insecurity for referred individuals and families.
• Complete engagement and assessment tools.
• Develop individualized, comprehensive care plans with goals using community resources.
• Provide care navigation within a Systems of Care framework in the DC/MD/VA region.
• Act as facilitator, advocate, liaison, and partner to connect individuals to needed supports.
• Identify services and resources to support intervention implementation.
• Address gaps in services and support while helping navigate barriers to resolution.
• Build understanding of regional services and establish partnerships for continuum of support.
