Job summary

JSSA’s Mental Health Services department is hiring a full-time Community Care Navigator to address economic insecurity across the DC, MD, and VA region. The role reports to the Community Care Navigation Team Leader and engages families to assess needs and coordinate comprehensive care plans using community resources.

The position requires a Bachelor's degree in social work or related field and 2–3 years’ experience navigating welfare systems, with proficiency in virtual work and Microsoft

Qualifications

• Bachelor's degree in social work, psychology, or a related field.
• Minimum of 2–3 years of experience in DC, MD or VA.
• Experience navigating social welfare systems.
• Proficient working in virtual environments and comfortable with supervisory and team support.
• Proficient computer/database skills (Microsoft Office) and at least 2 years of experience with an electronic health record or similar technology.

Responsibilities

• Triage and assess needs stemming from economic insecurity and complete engagement tools.
• Develop an individualized, comprehensive plan of care with goals and community resources.
• Provide care navigation within Systems of Care for assigned region(s) (DC/MD/VA).
• Serve as facilitator, advocate, liaison, and partner to connect to supports and services.
• Identify services and resources to support interventions.
• Address gaps in services and support and navigate barriers to resolution.
• Develop understanding of regional services and establish partnerships for continuum of support.
