Job summary

Prince William County is seeking a Community Navigation Specialist to coordinate services for residents with complex needs, collaborating with Fire & Rescue, the Area Agency on Aging, DSS, Police Crisis Intervention Team, behavioral health providers, healthcare partners, nonprofits, and other County departments to coordinate services.

The specialist facilitates multidisciplinary team meetings, supports coordinated case management, tracks outcomes, and helps identify gaps in service delivery.

Qualifications

• High school diploma or G.E.D. required; five (5) years of progressively responsible experience in case management, community navigation, human services, public health, behavioral health, emergency response, social services, or related field.

Responsibilities

• Coordinate services for residents with complex needs by leading multidisciplinary team meetings and coordinating case management.
• Track resident outcomes and identify gaps in service delivery; prepare reports demonstrating program effectiveness.
• Serve as lead administrator for the case management system, ensuring accurate data collection and longitudinal outcome monitoring.
