Job summary

AprilMay Inc is recruiting a Care Navigator to support children, youth, and families in accessing behavioral health, mental health, developmental, medical, school-based, and community services.

The role focuses on engaging families, identifying barriers to care, coordinating referrals, and assisting with appointment follow-ups while collaborating with clinicians, schools, providers, and internal teams to ensure timely support.

Qualifications

• Bachelor’s degree in a related field preferred.
• Experience working with children, families, or individuals with behavioral health, mental health, developmental, autism, or social service needs.
• Strong understanding of care coordination, family support, community resources, and social determinants of health.

Responsibilities

• Engage families and assess needs to connect them with behavioral health, mental health, developmental, medical and school-based supports.
• Identify barriers to access and coordinate referrals to appropriate services.
• Assist with appointment follow-up and connect families to community resources.
