Upper Peninsula Health Plan (UPHP) is seeking a Remote Community Based Clinical Coordinator - Care Manager to support members across the Eastern Upper Peninsula. The role requires frequent in-person visits to homes and facilities and periodic travel to UPHP’s Marquette, MI headquarters.

The position offers competitive pay, full-time remote work, comprehensive benefits, 401(k), loan repayment programs, and a $5,000 sign-on for eligible hires. RN licensure in Michigan is required.

Qualifications

• Licensed RN in Michigan with current license.
• Two years of clinical or health-related experience as RN or SW.
• Preferred: two years of managed care or five years nursing/social work experience.
• Valid driver’s license and personal vehicle for home visits.

Responsibilities

• Perform care management activities to assess, plan, and coordinate services for members.
• Greet and engage members during in-person visits in homes and facilities.
• Develop care plans with clear objectives and monitor progress.
• Coordinate with care teams, medical offices, and community services.
• Ensure compliance with MDHHS, CMS, NCQA, and other standards.
• Maintain confidentiality and participate in quality improvement activities.
