Upper Peninsula Health Plan (UPHP) is seeking a Remote Community Based Clinical Coordinator - Care Manager. The role involves care management duties, coordinating services, and meeting with members across the Eastern Upper Peninsula. Travel to UPHP headquarters in Marquette is required for onboarding and periodic training.

You will operate in a remote capacity with visits to homes and facilities, ensuring quality, cost-effective care while adhering to MDHHS, CMS, DIFS, and NCQA standards.

Qualifications

• Licensed RN in Michigan with active license.
• Two years of clinical or health-related experience as RN or SW.
• Valid driver's license with proof of insurance and a working vehicle.

Responsibilities

• Perform clinical functions following UPHP plans and regulations.
• Visit members in homes and facilities; collect health histories and assess needs.
• Develop care plans with clear objectives and interventions.
• Coordinate services and referrals across care settings to optimize outcomes.
• Collaborate with care teams to ensure proper discharge and follow-up.
