Job summary

Upper Peninsula Health Plan (UPHP) is seeking a Remote Community Based Clinical Coordinator - Care Manager. This full-time remote role supports UPHP members across the Western Upper Peninsula with travel to member homes and facilities as needed.

The position reports to Clinical Services and requires an Michigan-licensed RN with 2+ years of clinical or health-related experience. Compensation includes $29.72 per hour, full benefits, 401(k), student loan repayment programs, tuition reimbursement,

Qualifications

• RN licensed in Michigan
• Preferred: BSN or social work degree (licensed)
• Requires strong communication and organizational skills
• Proficiency with MS Office suite (Word/Excel) and keyboarding

Responsibilities

• Provide care management and coordinate medical and social services for UPHP members
• Conduct in-person visits to members in homes or facilities
• Develop and monitor individualized plans of care
• Collaborate with care teams and external providers
• Ensure compliance with MDHHS, CMS, DIFS and NCQA standards
