Eligible for travel to UPHP HQ in Marquette and periodic in-person visits. Requires a Michigan-licensed RN with 2+ years clinical or health-related experience; BSN or social work degrees preferred.

Qualifications

• Licensed RN required.
• BSN or licensed social work degrees preferred.
• Licensed in the state of Michigan.

Responsibilities

• Conduct in-person visits with members in homes and facilities.
• Assess health status and plan care with objectives and interventions.
• Coordinate services across care continuum and community resources.
• Monitor service delivery and referrals to support the care plan.
• Act as member advocate and collaborate with care teams.
• Maintain confidentiality and comply with regulatory standards.
