Qualifications

• Licensed RN with state of Michigan license is required.
• Two years of clinical or health-related experience as a licensed RN or social worker is required.
• Preferred: BSN or related social work licensure; Michigan MDHHS/CMS/DIFS familiarity is a plus.

Responsibilities

• Perform clinical care management duties to assess, plan, and coordinate services across the continuum of care.
• Gathers health history, monitors home environment, and coordinates community-based services.
• Serve as primary point of contact and advocate, coordinating with care teams and providers.
