Job summary

Concert Health is seeking a Collaborative Care Clinician II to deliver evidence-based behavioral health interventions via the Collaborative Care Model. You will work remotely with pediatric and adult patients, coordinating care with primary care providers, and engaging in ongoing supervision.

The role requires a Master’s in Social Work or related field and California licensure as an ACW, AMFT, or APCC, with 1–5 years of experience.

Qualifications

• Master’s Degree in Social Work, Counseling, or related field from an accredited program.
• Licensed to practice in the state of California as an associate clinician (ACSW, AMFT, APCC, or equivalent).
• Minimum 1–5 years experience as a licensed professional.
• A secure, high-speed internet connection capable of supporting video conferencing and cloud-based systems.

Responsibilities

• Provides care to patients, based on needs of assigned Care Pod, customizing treatment plan to individual patient needs.
• Supports and coordinates behavioral health care with the patient’s PCP and other service providers.
• Screens patients for behavioral health and substance use disorders using PHQ9, GAD7, and other tools.
