Job summary
Concert Health is seeking a Collaborative Care Clinician II, a licensed behavioral health clinician, to provide evidence-based interventions within the Collaborative Care Model for pediatric and/or adult populations.
Reporting to the Clinical Excellence Manager, you will coordinate care with primary care providers, monitor symptoms with PHQ9/GAD7, and document progress in Salesforce and the electronic health record to support integrated care delivery.
Qualifications
- Master’s degree in Social Work, Counseling, or related field.
- Licensed to practice in California as an associate clinician (ACSW, AMFT, APCC, or equivalent).
- Minimum 1–5 years experience as a licensed professional.
- A secure, high-speed internet connection capable of supporting video conferencing and cloud-based systems.
Responsibilities
- Provides care to patients based on the needs of the assigned Care Pod.
- Supports coordination with the patient’s primary care provider and other service providers.
- Screens patients for common behavioral health and substance use disorders using PHQ9, GAD7, etc.
- Monitors clinical symptoms and treatment side effects to reduce symptoms.
- Documents patient progress and treatment recommendations in Salesforce/EHR.
