Job summary

Concert Health is hiring a Collaborative Care Clinician II to deliver evidence-based care remotely to patients under a supervision framework. The role reports to Clinical Excellence Manager and uses the Collaborative Care Model to reduce symptoms and improve outcomes.

Minimum 1–5 years as a licensed clinician in California; bilingual English/Spanish preferred; the position emphasizes coordination with PCPs, patient education, and timely documentation in EHR systems.

Qualifications

• Master’s degree in Social Work, Counseling, or related field.
• Licensed to practice in California as an independent clinician (LCSW, LPCC or equivalent).
• 1–5 years of licensed professional experience.

Responsibilities

• Provide evidence-based interventions via Collaborative Care for adults and/or pediatrics.
• Coordinate care with the patient’s PCP and other providers.
• Monitor symptoms using PHQ9, GAD7 and other tools to guide treatment.
• Document progress and share plans with care team and collaborators.
