Job summary

Concert Health is seeking a CA-licensed clinician (ACSW/AMFT/APCC) to join our remote Collaborative Care team. The Clinician II provides evidence-based care to pediatric and/or adult patients, coordinating with primary care and specialists to optimize outcomes.

The role emphasizes supervision, bilingual capabilities, and utilization of Salesforce/EHR for treatment documentation, with competitive compensation and a comprehensive benefits package.

Qualifications

• Master’s Degree in Social Work, Counseling, or related field from an accredited program.
• Licensed to practice in California as an associate clinician (ACSW, AMFT, APCC, or equivalent).
• Minimum 1–5 years experience as a licensed professional.
• A secure, high-speed internet connection capable of supporting video conferencing and cloud-based systems.

Responsibilities

• Provides care to patients based on needs of assigned Care Pod, tailoring treatment plans to individual needs.
• Coordinates behavioral health care with primary care providers and other service providers as appropriate.
• Screens patients for behavioral health and substance use disorders using PHQ9, GAD7, CSSRS, or other tools.
• Monitors changes in clinical symptoms and treatment side effects with goal of symptom reduction.
• Supports psychotropic medication management prescribed by PCPs, focusing on adherence and safety.
• Documents patient progress and treatment recommendations in Salesforce and electronic health records.
• Educates patients about behavioral health and treatment options.
• Establishes mutually agreeable care management plans with the patient and care team.
