Job summary
Concert Health is hiring a Collaborative Care Clinician II for a 100% remote role in the US. The position requires an MSW/MLC with California licensure (LCSW/LPCC) and 1–5 years of licensed experience.
The clinician will deliver evidence-based interventions under the Collaborative Care Model and coordinate multi-disciplinary care for pediatric and/or adult patients. Strong English/Russian medical interpretation, excellent documentation, and proficiency with EHRs are essential.
Qualifications
- Master's degree in a related field is required.
- Licensed to practice in California as an independent clinician (LCSW or LPCC).
- Minimum 1–5 years of licensed professional experience.
Responsibilities
- Provide evidence-based interventions via Collaborative Care Model.
- Coordinate care with primary care and other providers.
- Screen patients for behavioral health and substance use disorders.
- Monitor symptoms and treatment side effects with goal of symptom reduction.
- Support psychotropic medication management and adherence.
- Document progress in Salesforce/EHR for sharing with care team.
- Educate patients about behavioral health and treatment options.
- Plan mutual self-management goals with patients.
- Participate in supervision and caseload consultations.
- Identify social determinants barriers and assist access to care.
