Unleashing Leaders is seeking a Transformative Care Model Clinical Adoption Support Lead to drive healthcare transformation. This role partners with providers, health systems, and community organizations to support adoption of new care models and workflows in clinical settings.

You will develop adoption strategies, build relationships, assess readiness, deliver training, monitor adoption metrics, and report risks and opportunities. Hybrid role in Sacramento area with strong benefits.

Qualifications

• Bachelor’s degree in nursing, public health, healthcare administration, or related field.
• 5+ years of healthcare experience with clinical teams or delivery organizations.
• Ability to translate clinical concepts into practical workflows and training.

Responsabilités

• Develop adoption strategies that support clinical and healthcare transformation initiatives.
• Build trusted relationships with providers, clinicians, and health systems.
• Assess readiness and tailor adoption plans for rural and diverse program needs.
• Serve as SME and resource to grantee implementation teams.
• Develop communication toolkits and resources for adoption.
• Facilitate workshops and training sessions for clinical and operational audiences.
• Monitor adoption metrics and address barriers to change.
• Establish KPIs and report adoption progress, risks, and opportunities.
