CVS Health is seeking a Practice Transformation Advisor to elevate the quality and efficiency of care across our provider network. You will partner with large practices to implement data-driven quality strategies and guide staff through training and operational improvements.

The role emphasizes data analytics, cross-functional collaboration, and adherence to regulatory standards, with a remote, full-time arrangement and a comprehensive benefits package.

Qualifications

• Bachelor’s degree or commensurate work experience in healthcare administration, public health, or a related field.
• Licensed Allied Health Professional, or Licensed Registered Nurse per preferences.
• Certified Six Sigma Green Belt or CPCH/quality certification preferred.
• CPHQ or equivalent certification preferred.

Responsabilités

• Engage large provider groups (1,000+ members) and foster engagement.
• Analyze provider performance metrics and identify improvement opportunities.
• Design and deliver training for primary care and specialty providers and staff.
• Lead performance improvement initiatives with cross-functional teams.
• Conduct audits and provide actionable feedback for accuracy and quality.
• Collaborate with clinical, operations, and provider relations teams to align objectives.
• Collect and report performance data via dashboards for leadership review.
• Stay current on industry trends to support population health and equity efforts.
