Job summary

Post Acute Analytics is seeking a Clinical Program Manager to operationalize our clinical programs across health plans and provider partners. You will translate clinical strategy into executable workflows, coordinate implementation, adoption, and ongoing performance monitoring to ensure clinical actions move to completion.

Ideal candidates have 5+ years in healthcare program implementation or related areas, with strong analytical and communication skills, and the ability to manage multiple

Qualifications

• Bachelor's degree in nursing or a related field; active license strongly preferred.
• Five+ years of healthcare experience with clinical program implementation, utilization management, health plan operations, or related settings.
• Working knowledge of payer and provider workflows in regulated healthcare environments.
• Ability to manage multiple workstreams, stakeholders, risks, and deadlines independently.
• Strong written and verbal communication with ability to translate findings into practical recommendations.

Responsibilities

• Translate clinical program strategy into implementation plans and workflows.
• Coordinate readiness, education, launch activities, and stabilization across assigned programs.
• Develop recurring program reporting linking adoption, performance, clinical measures, and client priorities.
• Collaborate with Product and technical teams on workflow needs and clinical requirements.
• Communicate progress, risks, decisions, and performance to stakeholders.
