Optum Insight is seeking a Clinical Pharmacist to join the ACO Practice Extend team. You will manage medication therapy for patients, optimize regimens, and coordinate follow-up with providers to improve outcomes and reduce cost of care.

The role offers remote work from anywhere in the U.S. with a schedule that may include two evenings per week. Licensure in multiple states may be required; strong communication and clinical knowledge are essential.

Qualifications

• Pharm.D. degree from an accredited school.
• Current pharmacist licensure in state of residence in good standing.
• Active Florida pharmacist licensure in good standing.
• Ability to obtain additional state licensures as needed.
• 3+ years of direct patient care MTM/CMR experience in a clinical setting.
• Spanish/English bilingual.
• Ability to work evenings (up to 7pm local time).
• Strong communication with patients and providers.
• Robust knowledge of guideline-directed disease state management.

Responsabilités

• Conduct Comprehensive Medication Reviews for high-risk patients and referrals.
• Perform Targeted Medication Reviews for high-cost meds with lower-cost alternatives.
• Coordinate follow-up with Care Team for patient and provider engagement.
• Apply MTM skills to optimize therapies and reduce adverse events.
• Communicate actionable drug-therapy recommendations to providers.
• Interpret clinical information, MTM data, and benefit information in EMRs.
