Optum Insight is expanding its Practice Extend Care Team by adding a Clinical Pharmacist. You will lead medication therapy management, conduct comprehensive and targeted reviews, and collaborate with RN Care Coordinators to improve patient outcomes and reduce cost of care. The role supports remote work from anywhere in the U.S.

with evenings availability, and requires Pharm.D. and active pharmacy licensure.

Qualifications

• Pharm.D. degree from an accredited school.
• Current Pharmacist licensure in good standing.
• 3+ years of direct MTM and CMR experience.
• Spanish/English bilingual ability.
• Flexibility to work evenings (until 7pm local).

Responsabilités

• Work with a Care Team to support patient and provider engagement for Care Coordination and MTM.
• Conduct Comprehensive Medication Reviews (CMRs) for High Risk patients.
• Conduct Targeted Medication Reviews (TMRs) for high-cost meds.
• Triage patients to Clinical Care Coordinator for follow-up help.
• Apply MTM skills to optimize outcomes and cost-effectiveness.
• Communicate actionable, patient-specific drug recommendations to providers.
• Use EMRs and pharmacy systems to interpret clinical and benefit info.
• Document MTM interventions accurately in the tech platform.
• Present patient case examples to program leadership and external providers.
