UnitedHealth Group is seeking a Clinical Pharmacist to support medication therapy management within the Optum Insight ACO Practice Extend program. This role partners with a Care Team to optimize therapy, reduce adverse events, and improve outcomes across patient populations.

The position offers remote work from anywhere in the U.S. and requires Pharm.D., active licenses in required states, and MTM experience.

Qualifikationen

• Pharm.D. degree from an accredited School of Pharmacy.
• Current pharmacist licensure in state of residence in good standing with no ongoing disciplinary actions.
• Active Florida pharmacist licensure in good standing.
• Ability to obtain additional state licensures based upon business need.
• 3+ years of direct patient care experience conducting Medication Therapy Management and Comprehensive Medication Reviews in a clinical setting.
• Spanish/English bilingual.
• Ability to work flexible schedule, including two evenings (up to 7pm local time).
• Ability to communicate effectively with patients, engage providers, collaborate with team members, and present to internal team and/or external providers.
• Knowledge of pharmacy benefits, drug utilization review, and formulary management.

Aufgaben

• Work with a Care Team of a Care Coordinator and Clinical Administrative Coordinator to support patient and provider engagement for Care Coordination and MTM.
• Conduct Comprehensive Medication Reviews (CMRs) for High Risk patients and those referred by PCPs.
• Conduct Targeted Medication Reviews (TMRs) for high-cost medications with lower-cost alternatives.
• Triage patients to Clinical Care Coordinator for follow-up assistance.
• Apply clinical knowledge to avoid drug therapy problems and optimize outcomes.
• Engage providers with actionable, patient-specific drug-therapy recommendations.
• Utilize EMRs and other platforms to interpret clinical information, claims, and benefits.
• Document MTM interventions accurately within the technology platform.
• Present patient case examples of MTM interventions to leadership and external providers.
